Choices 65 (HMO)

Provider Network A provider network includes the hospitals, physicians and other healthcare providers who provide healthcare services to Choices 65 (HMO) members. These other healthcare providers are commonly called “ancillary providers,” and include urgent care centers, diagnostics facilities, long-term acute care centers and skilled nursing facilities, among others. Choices 65 (HMO) also has certain pharmacies that we have contracted with to provide you with your prescription drugs.
