Things You Should Know
Choices Select (HMO-POS) is not a standard Medicare health plan. Unlike other Medicare health plans, we are a triple-option plan. This means that as a member of Choices Select (HMO-POS), you have the flexibility to have your care coordinated by your own primary care physician (PCP). Or, you can see any physician in the Choices Select (HMO-POS) network of physicians without a referral from your PCP. You have a third option of going to any doctor or specialist outside of the Choices Select (HMO-POS) network and still receiving Medicare-level benefits.
With Choice 1 (Within Physician Team), you benefit from the guidance and expertise of your PCP – a healthcare professional who knows your medical history and lifestyle.
Most of our members prefer having their medical care coordinated within their physician team, which is composed of PCPs and specialist physicians. Members can benefit from the guidance and expertise of working with their PCP, who can help coordinate care with the doctors and hospitals in their physician team. Choice 1 maximizes your savings. Office visits to your PCP cost just $5, and office visits to specialists within your physician team are just $15.
With Choice 2 (Within Choices Select (HMO-POS) Network), you enjoy the convenience of going to any physician within the Choices Select (HMO-POS) network.
Choices Select (HMO-POS) members can visit any primary care physician or specialist they choose within the Choices Select (HMO-POS) network of physicians. They have the choice to access care when and how they wish. You can see any physician in the Choices Select (HMO-POS) network of physicians, without a referral from your PCP, for only $25.
Choice 3 (Out-of-Network) gives you complete freedom by allowing you to go outside the Choices Select (HMO-POS) network to see any doctor or specialist you choose and still receive Medicare-level benefits.
With Choice 3, you can keep your current doctors and your Medicare benefits! If your current doctor isn't in the network, you can still get Medicare-level coverage to see your doctor with Choices Select (HMO-POS) through Choice 3. If you want to use our healthcare providers for some services but decide, at any time and for any reason, to use a physician or hospital outside the network, you'll still have the same coverage as Medicare. Choice 3 has higher costs than Choice 1 or Choice 2. You pay 20% of the total costs when you see any physician or other healthcare provider who is not part of the Choices Select (HMO-POS) network.
Choices Select (HMO-POS) gives you the flexibility to customize your care to meet your changing needs and to change your mind anytime you want without having to change coverage. With Choices Select (HMO-POS), you have more control over the healthcare you receive. All three choices are available to you at any time, though higher costs may apply when you receive care from a physician outside the network using Choice 3.
