Plan Details

Peoples Health Group Medicare (HMO-POS) is a triple-option health plan. As a member, you can access your benefits in one of three ways:

• Within Physician Team - You select a primary care physician (PCP) from the Peoples Health provider network when you enroll. Office visits to your PCP are $5, and your visits to specialists within your physician team are only $10.
  
  
• Within Peoples Health Group Medicare (HMO-POS) Network - You can see any specialist in the Peoples Health Group Medicare (HMO-POS) network without a referral from your PCP with for only a $25 copay for each visit.
  
  
• Out-of-Network - You can go to a non-participating or an out-of network provider without a referral from your PCP and still receive Medicare-level benefits. You will pay 20% coinsurance.

You receive more benefits with Peoples Health Group Medicare (HMO-POS) than with Original Medicare, including:
