Eligibility and Enrollment

Eligibility Guidelines
Determining if you're eligible to enroll in Peoples Health Group Medicare (HMO-POS) is easy! In fact, if you satisfy these enrollment requirements, you're eligible to become a Peoples Health Group Medicare (HMO-POS) member.

To be eligible for Peoples Health Group Medicare (HMO-POS), you must:


• Be a retiree with an employer that is contracted with Peoples Health
• Be enrolled in Medicare Part A and Medicare Part B
• Not have ESRD (End-Stage Renal Disease), with limited exceptions, such as if you develop ESRD when you are already a member of a plan that we offer, or you were a member of a different plan that was terminated
• Reside in one of the following Louisiana parishes:

- Ascension
- East Baton Rouge
- Jefferson
- Livingston
- Orleans
- Plaquemines
- St. Bernard
- St. Charles
- St. James
- St. John
- St. Tammany
- Tangipahoa
- Washington
- West Baton Rouge

What to Expect Next
Here's a list of things to expect as a new Peoples Health Group Medicare (HMO-POS) member:

New Member Enrollment Call
Upon receiving your enrollment request form, a Peoples Health representative will call you to verify that the information on your enrollment request form is correct and that you understand your benefits and plan structure.

New Member Packet
As a new member of Peoples Health Group Medicare (HMO-POS), you will receive a new member packet in the mail within a few weeks of your enrollment. This packet contains important information about your coverage through Peoples Health Group Medicare (HMO-POS). Be sure to read everything in this packet and call us with any questions.

Health Risk Assessment Call
In an effort to maintain and even improve your well-being, we conduct a health risk assessment of each new member. During the assessment, we will ask you questions about your health so we can determine your overall well-being. This way, we can customize your care to best fit your medical needs.

Transition Call
Once you become a new member, we may need to transition your care to ensure that you continue to receive the medical treatment you need. By doing so, you will have uninterrupted access to any medications you are currently taking and any durable medical equipment you may be currently using.
